Heritage Hospice is available to those who:
1) Have been diagnosed with an illness that cannot
be cured with a life expectancy of six (6) months or less;
2) In agreement with their physician, desire to
receive Hospice care;
3) Desire to receive compassionate, professional
care in the comfort of their own home or home like setting, including nursing
home; and
4) Live in the counties of Boyle, Garrard, Lincoln,
and Mercer.
What is Special About Hospice Care?
Hospice care assists patients and their families
to live their lives to the fullest extent possible. This is done
by relieving physical pain and other discomfort and by providing
emotional support to patients and their loved ones. Hospice care
enables patients to remain at home for as long as possible, surrounded
by people and things they care about the most.
Who is Eligible for Hospice Care?
Patients who are living with an advanced life-limiting
illness with a prognosis of 6 months or less are eligible for care.
Hospice services are designed to assist patients living with a variety
of illnesses and conditions including but not limited too, cancer, heart
disease, stroke, pulmonary disease, Alzheimers, and dementia.
Where is Hospice Care Provided?
Hospice is not a place. It is a philosophy
of care that can be provided wherever a patient/family call home.
This may include their private residence, nursing facility, assisted living
facility or hospice inpatient facility.
When Is It Time To Begin Conversation with Patients and Families
About Hospice?
At any time. The sooner discussions can occur
the better. Advance care planning is beneficial when the patient
can be a part of the discussions and share their wishes with loved ones
and their health care provider.
