Who can use Transitions?
Transitions is a community service offering information
and referral assistance, encouragement and support through case management
and volunteer services. Anyone experiencing a life-limiting illness is
eligible for services once consent is given by their primary physician.
People receiving Transitions can be undergoing curative as well as palliative
treatment.
Transitions Services.
Case Management. To help identify your needs and
provide assistance to help you meet those needs. The Transitions Coordinator
can help link you to community support services and information and referral
sources so that you and your family will not feel quite so overwhelmed.
Volunteer Services. Trained volunteers donate their
time and give assistance, encouragement and support through friendship,
light housekeeping, help with errands, companionship, respite "breaks"
to caregivers.

The mission of Transitions is to provide
volunteer and case management services to individuals with a terminal illness
and their families who have a prognosis of one year or less to live.
The Transitions program will be offered to all appropriate
individuals and their families without regard to race, creek, color, sex,
age, national origin, religious background, physical disability, veteran
status, medical treatment, or the ability to pay.
Furthermore, Transitions also believes that bereavement
support is very critical to one's mental health and well being. Bereavement
services will be made available to all Transitions families, whether or
not they are admitted to Hospice care.
Funding.
Transitions is funded by Heart of Kentucky, United
Way, memorial/honorary, and private donations.
