Heritage Hospice, Inc. provides a multidisciplinary approach to care that includes the patient and family. Our team includes physicians, registered nurses, madical social workers, and more.
Palliative care involves addressing physical, intellectual, emotional, social, and spiritual needs, as well as, facilitating patient autonomy and access to information and choice.
For up to 13 months following the death, specially trained hospice bereavement staff and volunteers are available to provide assistance to family members dealing with grief and bereavement.
