How to Make a Referral

Anyone may make a referral or request more information about hospice services, but a physician must agree to the care and certify that the patient meets the eligibility requirement of having a life-limiting illness. To make a referral, please contact us at 859-236-2425.

Heritage Hospice, Inc. will contact the patient’s doctor to make sure he or she agrees hospice care is appropriate. If the physician agrees, the hospice admission staff will visit with the patient and family/caregiver to discuss services and admit to care.

Hospice care is intended to supplement the care provided by the family and caregivers, not to replace it.

When Should You Call Hospice?

Anytime. Being informed about services available is an important step to receiving the care you or a loved one may need, exactly when needed most.

It is often said by families of patients served by hospice, “I wish we had known about you earlier” and “I wish we had hospice care much sooner than we did. What a difference you made in our lives!”

Hospice serves all ages and care is provided in the home, nursing homes, and assisted living centers.
Hospice admits patients for any illnesses, including cancer; end-stage lung, liver, kidney, and heart diseases; and Alzheimer’s.

What Does Hospice Cost?

Hospice is covered by Medicare Part A, Medicaid, and most commercial insurance plans. Care is provided to all patients meeting eligibility criteria regardless of ability to pay.

Services will be provided to those who meet the admission criteria. Heritage Hospice, Inc. does not discriminate in the admission of patients because of race, color, religion, sex, national origin, age, language, handicap, or financial position.
