Hospice

What services are offered?

Heritage Hospice, Inc. provides a multidisciplinary approach to care that includes the patient and family.

The hospice team includes:

  • The physician works with the hospice team and provides oversights to the patient’s plan of care.
  • The hospice medical director works with the patient’s own physician and hospice team to provide care and support to the patient according to the plan of care.
  • Registered nurses visit the patient regularly to monitor the patient’s medical condition and to make sure the patient is as comfortable as possible. A nurse is available to answer questions and visit 24/7.

  • Medical social workers provide support, counseling, and information/referral services to the patients and family.
  • Chaplains provide spiritual support and counseling to patients and families, but do not replace their own pastoral support.
  • Hospice aides provide personal care and light housekeeping services.
  • Bereavement counselors provide support and counseling up to 13 months following the death of a patient.
  • Volunteers offer companionship and support to patients and family.
  • Registered dietitians, physical therapists, occupational therapists, and speech language therapists are available as needed.
