Hospice

Heritage Hospice, Inc. provides a multidisciplinary approach to care that includes the patient and family.

The hospice team includes:

The physician works with the hospice team and provides oversights to the patient’s plan of care.
The hospice medical director works with the patient’s own physician and hospice team to provide care and support to the patient according to the plan of care.
Registered nurses visit the patient regularly to monitor the patient’s medical condition and to make sure the patient is as comfortable as possible. A nurse is available to answer questions and visit 24/7.

Medical social workers provide support, counseling, and information/referral services to the patients and family.
Chaplains provide spiritual support and counseling to patients and families, but do not replace their own pastoral support.
Hospice aides provide personal care and light housekeeping services.
Bereavement counselors provide support and counseling up to 13 months following the death of a patient.
Volunteers offer companionship and support to patients and family.
Registered dietitians, physical therapists, occupational therapists, and speech language therapists are available as needed.
